Request for Certificate of Eligibility Form DS-2019: The DS-2019 is a federal form issued to certain categories of scholars/lecturers by the Center for Global Engagement under the US Department of State J-1 Exchange Visitor Program guidelines.

For use by academic departments to request a DS-2019 Certificate of Eligibility for an international research scholar, visiting short-term scholar or professor, and in some cases, non-degree seeking exchange students, who temporarily come to FSU for research, teaching, lecturing, consulting, or participating in colloquia.
